RESERVATION BOOKING FORMS
Please download and print the following three (3) Reservation Booking Forms found below. All three (3) forms must be signed, scanned, and emailed to dtatooles@southerncrossgalleries.com along with payment of the requisite 30% deposit to secure your reservation. If you are unable to scan and submit the completed Reservation Booking Forms by email, the forms may be submitted by regular mail. Please refer to our Contacts Page for our mailing address. We will also gladly email you hard copies of the Reservation Booking Forms if you would rather not work online.
• Liability Release and Assumption of Risk Agreement
• Travel Insurance Acceptance Form (required at or before final payment due date)
Adobe Acrobat Reader:
Southern Cross Galleries Photo Safaris Booking Forms have been created using Adobe Acrobat software. The forms are in Adobe Portable Document Format (PDF). To open the forms you must have Adobe Acrobat Reader software installed on your computer. Adobe distributes a free Adobe Acrobat Reader for PDF files at its web site and it is most likely already installed on your computer as a plug-in to your web browser software.
